Applicants to Deacon Formation Program

It is necessary to obtain a PA State Police Record Check and a PA Department of Welfare Child Abuse Record Check as part of your application process.

This is a standard requirement for anyone who will work with children and is required of all clergy and those in formation for the priesthood and diaconate. This link will take you to an information page that will explain how to obtain the Police Record Check.

After you obtain the record check go to this page for information on how to go about obtaining the Child Abuse Record check. This can be applied for online. The police record check is required before the Department of Welfare will process your Child Abuse Record check.

If you have any questions call Karen Street at 814-824-1222.
