New Teacher Registration Form for August Orientation Please complete the linked survey for all teachers newly hired in the Diocese of Erie. They must attend the New Teacher Orientation at the St. Mark Catholic Center on August 14 or on the alternate date in September. Attendance for the whole day is contractually required of all new teachers, full-time and part-time, including those who were hired after the September Orientation last year (see Teachers’ Contract). NWEA Fall Student Roster Files

Please upload the student roster files for the upcoming Fall NWEA testing window. Click here for directions.

Student IDs must be the same from year to year.

New students need a new ID with the corresponding six-digit ID with school prefix.

  • Check their permanent record folder for a potential NWEA ID
  • If a student has an existing ID, the old ID should be placed in the previous student ID column and the new ID that corresponds to the new school should go in the student ID column.

Verification of Instructional Hours

Schools who wish to utilize Flexible Instruction Days must first submit Verification of Instructional Hours and a copy of the procedures for use to the Assistant Superintendent for approval.

Annual Assurance Form - Staff

You must also submit the corresponding list of employees and volunteers.

Following are excerpts from policy 401.11.

On an annual basis schools will provide to the Assistant Superintendent for School Personnel an assurance (Appendix 401.11A) that all required valid clearances, all state and diocesan mandated training certificates, and signed Ethical and Moral Guidelines are maintained in the personnel files of all current employees.

On an annual basis schools will provide to the Assistant Superintendent for School Personnel an assurance (Appendix 401.11B) that all required valid clearances and all state and diocesan mandated training certificates are maintained in the files of all current volunteers. A comprehensive list of documentation for inclusion in a volunteer file can be found on the Office of Protection for Children and Youth website.

STAG Tuition Assistance Verification Student Tuition Assistance Grant (STAG) awards are reserved for PK-8 (halftime or greater) school employees, with children who attend a K-12 Catholic school within our diocese. Fall Workshops

Fall Differentiated STAGR Workshops 2025 - September 29 - October 10 at various locations throughout the Diocese. Register at WisdomWhere. Each day/building has a separate link. Share information about the fall workshops via your school newsletters with your school communities. Email the Superintendent letter to your teachers regarding the workshop.

Administrator’s Receipt of Policies

The school policies are updated annually. You will be notified in the CSO Brief when they have been updated. Receipt of Policy forms submitted prior to the updates will not be accepted.

Administrators and all employees must review the Policies for Catholic Schools in the Diocese of Erie. All staff must be given sufficient time to read and seek clarification. Administrators must require all paid or stipend employees to acknowledge they have read, understood, and will adhere to diocesan policies by signing the Receipt of Policies for Catholic Schools in the Diocese of Erie form. Annually, building principals are asked to collect and retain these signed forms and retain the original in the employee personnel file. Building principals must sign and submit the Administrator's Receipt of Policies for Catholic Schools in the Diocese of Erie.

Documentation 2 Step Process

Step 1:

Every school year, schools must collect and retain a signed Certificate of Individual Request for Loan of Textbooks, Instructional Materials and Equipment from every family with children enrolled in grades K to 12 and who are residents of Pennsylvania.

A sample letter and the certificate to be sent home to parents or guardians can be found through the link above. The certificate on the website is provided by PDE and my not be altered or included in a student application form. Currently, electronic signatures are not accepted on the certificate.

Step 2:

The principal must verify that a signed certificate is on file for every Pennsylvania family and on school letterhead, write a letter certifying all certificates are on file. A sample letter for each school is linked above. After submitting the letter to the CSO, keep the original on file.

CIPA Annual School Checklist

A fillable form is located in the Government Programs section.

To accurately complete this form, the following actions must occur to be in compliance with Policy 202.2 Acceptable Use of Internet, Computers, and Network Resources:

  • Disseminate your Internet Safety Policy to all stakeholders annually. (Annual date of dissemination must be included in your CIPA checklist)
  • Ensure Acceptable Use of Internet Safety Policy Agreements are signed annually and are on file for all students, teachers, staff, and volunteers.
  • Maintain a log and record of the annual Digital Citizenship instruction provided for all students.
  • Maintain technology protection measures through use of Internet Filtering software.

Annual Overview of Professional Development

The form includes the list of required professional development days and training per CSO policies, Bishop’s Expectations, and state requirements.

Annual Required Professional Development and Trainings:

  • Spiritual Journey for Elementary and Middle Schools - (Contractual) – this day is separate from a faculty retreat day
  • Faith Formation for Secondary Schools – (Contractual) – this day is separate from a faculty retreat day
  • Suicide Awareness and Prevention (See Policy #302.9) – 1 hour per year
  • Trauma-Informed Education (Act 18 of 2019) – 1 hour per year
  • CPR/AED (See Policy #303.7) – offered every 3 years – schools are required to have at least one person on the premises during regular school hours who is currently certified in CPR
  • Universal Precautions (See Policies #202.3 & #303.7) – review annually at the initial faculty meeting

The Office for the Protection of Children and Youth also has requirements per their policies.

Professional Development Needs Assessment Survey 

A needs assessment survey must be completed annually by all teachers and administrators for:

  • The CSO to be an Act 48 Provider
  • Schools to access government funding for professional development and student resources.

Annual Catholic School Report for Child Protection

This report is to be completed by all schools. The Office of Protection of Children and Youth have an annual diocesan Child Protection Compliance Audit mandated by the United States Conference of Catholic Bishops. 

The report and further information will be included in a CSO Brief (usually in May).
